Create

This form is divided into steps. The step the user is on is marked by the number of the step turning orange. Once the step is filled with the required information, the circle marking the step will stay orange and the number in said circle will change into a check mark (✓) .

1. Select a patient → The first step involves searching and selection of a patient, the search grid can be narrowed down with the search field. Once a patient is selected, the main and important information about this patient will be displayed at the bottom of the form in a table format. Until the patient has been selected, the user cannot continue to the next step. To continue to the next step, the user must press the next button found at the bottom right of the form.

1. Basic information → To complete this step, only the birthdate is required. To input this information, the user has two options. They can fill in the information with the format given, or they can click on the little calendar icon, , and select the correct date. Once this information is filled in, the box next to it will automatically be filled with the patient's current age.

1. Analytical tests → One phone number must be assigned to a patient for further contact.

1. Biological samples → This step has no required fields, but it contains a checkbox at the bottom, Add provenance region?. This option is for when a patient, although local, has come from outside the country. When the box is ticked, two more boxes appear on the forms, Provenance region and Provenance city.

The next button cannot be used if any of the required fields are not filled in. In every step, there is a Previous button for the user to be able to navigate back and forth between steps and alter the information they contain.

Search historic

At the bottom left of this tab, there is a download button. Depending on the status of the checkboxes in the downloaded fields tab, different downloading options may be presented to the user.
